?T Passport Renewal

General Information
l      
Appointments are not required.
l      
Residents in GA, AL, NC, SC or VA who reside beyond 100 miles from Atlanta, they can apply for a passport through the mail. But they will have to come to the Consulate Office in person to pick up the new passport.
l      
Passport applications take 1 week to process.
l      
We cannot process or deliver passports by mail.
l      
We cannot accept an application form by fax or a copy of an application form
l      
Children now have to have their own passports. Children need a 5 year passport application form.

You are required to submit the following:
l      
Application Form
  You must fill out the application form using black ink. 
It must be filled out correctly with applicant's signatures. Even if applicants are elementary students, their signature is necessary for applications.

l      
Family Register, Koseki-Tohon or Koseki-Shohon in Japanese, issued within last 6 months
  The Family Register is not required if you have fulfilled the following requirements.
    1.       You have registerd Zairyu-Todoke to the Consulate in Atlanta at least 3 months ago.
   
2.      You have a valid passport.
l      
Photographs can be black and white or color, but the length must be 4.5 cm and width must be 3.5 cm (1-3/4" x 1-3/8"). The size of your face in the photo should be about 2.7 cm from the top of your head to the chin, and must be facing forward. We cannot accept photos in which you are wearing sunglasses or a hat, nor any photos with shadows, background obstruction such as curtains, flash reflection from glasses, or a photo taken in a quick photo booth. If the photo does not concur with the regulations, we may ask for the photo to be retaken.
l      
Your current passport
l      
The copy of your states in the U.S. such as:
    1.       Green card
    2.      I-20 IAP-66 form
    3.      Visa
    4.      Birth Certificate or U.S. passport if you are born in the U.S.

l      
The processing fee is $136.36 for a Ten-year valid passport and $90.90 for a Five-year valid passport, which must be paid by cash when you pick up the passport. The processing fee for children under 12 years old is $45.45.
l      
Please note an application for minors under 20 years of age needs a parent's signature on the form or signed parent's consent form.
l      
When you're married to a non-Japanese and applying a passport with the married name for the first time, you have to submit a family register issued within the last six months in which your marriage is recorded. We also request a driver license, green card, or marriage certificate to confirm the spelling of the last name.

Other Information
l      
Please be prepared to write the address of your registered domicile in Japan as well as the contact name and address of family members or relatives.
l      
Your surname will remain unchanged unless you request a surname change.


?U Passports for Infants

General Information
l      
Children now have to have their own passports. Children need a 5 year passport application form.
l

Please bring your child with you when submitting the application form.
l

You are required to submit the following:

To apply for a baby's passport for the first time, you need to bring your child's family register issued within the last six months, along with two photographs and proof of the parent's current address, such as a driver's license or a checkbook.
l      
Photographs can be black and white or color, but the length must be 4.5 cm and width must be 3.5 cm (1-3/4" x 1-3/8"). The size of the face in the photo should be about 2.7 cm from the top of the head to the chin. Please bring photo without shadows or background obstructions such as curtains. If the photo does not concur with the regulations, we may ask for the photo to be retaken.
l      
The name should be spelled in the Hepburnian system, but if either parent is a non- Japanese citizen, you may submit a request to spell without using the Hepburnian system which may enable you to use the correct spelling. In this case you must bring such a document as a U.S. birth certificate to confirm the spelling.
l      
The processing fee is $45.45 which must be paid by cash when picking up the passport.

Other Information
l      
Please be prepared to write the address of your registered domicile in Japan as well as the contact name and address of family member or relatives living in Japan, since these are required in the application.

Lost Passports

General Information
l      
If you have lost your passport, you must apply for a reissue of your passport.
l      
Appointments are not required.
l   
  Residents in GA, AL, NC, SC or VA who reside beyond 100 miles from Atlanta, they can apply for a passport through the mail. But they will have to come to the Consulate Office in person to pick up the new passport.
l      
It will take approximately two or three weeks to reissue the passport, since we must confirm the replacement of the passport with the Ministry of Foreign Affairs in Tokyo.
l      
We cannot process or deliver passports by mail.
l      
We cannot accept an application form by fax or a copy of an application form.

You are required to submit the following:
l      
Two photographs
l      
Police Incident report from the police confirms the loss of your passport.
l      
The reissue processing fee is $109.09 for a Ten-year valid passport, $72.72 for a Five-year valid passport, and $36.36 for applicants under 12 years of age.
l       In case the lost passport has already expired, you must bring your family register issued within the last 6 months.  In this case, the processing fee will be $136.36 for Ten-year valid passport,  $90.90 for a Five-year valid passport and $45.45 for children under 12 years old, which must be paid by cash when you pick up the passport.
l       Please note an application for minors under 20 years of age needs a parent's signature on the form or signed parent's consent form.

Other Information
l      
If you need to return to Japan immediately, PLEASE CALL US FIRST. We may  issue a travel document for your return to Japan. In this case, you must bring a police theft report, two photographs, your airline ticket to Japan, and proof of your Japanese citizenship such as your family register or your Japanese driver's license. The processing fee is $22.72.
l      
Because the travel document is valid only for a week after its issue, please apply within a week of your return to Japan.
l
